There may be a time when you are unable to make your own healthcare choices. Advance Care Planning (ACP) is a process for you to plan for future healthcare situations, including who will make choices for you if you are unable. An Advanced Medical Directive must be signed by two adult witnesses or a notary public. If you can’t physically sign your document, the notary or the person who signs it for you cannot be a witness. If you use witnesses instead of a notary, one of the witnesses cannot be your spouse or relative. (Source:UCOA) (utahaging.org)
